a pdf document and and includes a Standing Order mandate. If you don't
have a pdf (Adobe Acrobat) reader then for a free down load click Adobe
Acrobat pdf Reader
- Print the pdf document.
- Fill your name in the box provided. This will then form your personalized
Email address. For example Mark Smith will be given mark.smith@mail4me.co.uk
- Fill in the rest of your personal details on the first page and your
bank details on the second page
- Send the completed document to the address shown
